Instructions for downloading and printing the visa application form

1. The file dubaivisa.pdf consists of five pages.
2. Please print back-to-back on A4 size paper.
3. Pages 1 to 4 (two sheets printed back-to-back) consist of the application form to be completed and signed by each applicant. Please staple these pages together before completing.
4. Page-5 is a template to be used only if you are applying by courier. Use cloth-lined envelope 12 cm x 31 cm only. Take a printout of page-5, cut around the boundary line, and paste on the front of the cloth-lined envelope.

Guidelines for completing the visa application form

1. Remember - one application set for each applicant (including infants).
2. Write "NA" where Not Applicable.
3. Item 3, to be completed only if you opt for return of passport by courier. Please provide the address to which the passport is to be delivered: must be either your CURRENT address, or address of your travel agent.
4. At item 7, please describe your occupation in some detail - e.g. "Teacher", "Manager in an ad agency", "Software engineer" etc.
5. It is recommended that you attach your business card to the form.
6. All applicants aged 14 and above must sign the application form on page-3. Where applicant is below 14, a parent must sign.

Visa fee per person (including infant) at current exchange rates: -
 a. Tourist Visa (30 days stay): Rs. 3700/- (Includes visa fees + service tax)
 b. 96 Hour Visa (96 hrs stay): Rs. 3050/-
(Includes visa fees + service tax)

1. DD should be drawn on Mumbai favouring "DVPC A/C ABN Amro 2537893".
2. DDs drawn on co-operative banks are NOT accepted.
3. A family travelling together can send one DD for the consolidated amount - include the DD with application from Principal Applicant.
4. Please note, visa fee is non-refundable.
